REGISTRATION INFORMATION

If you plan to attend, you can fill in the  Registration Form below or you can download the pdf file of the registration form. Registration should be done no later than 20 May 2001.
Please send a separate form for each participant.

ON-SITE   REGISTRATION

All delegates must be registered at the registration desk.
Registration of delegates will start at 9.00 on 12 June and it will continue the following day at the on-site registration desk.

CANCELLATION AND REFUND POLICY  
A refund, less 20% deduction for administrative costs, will be issued for written cancellations received by 20 May 2001. For delegates who notify their cancellation after 20 May 2001 or  will not attend the congress, a charge of 50% of the congress fee will be withheld and a copy of the proceedings will be sent after the event.
